If your child requires any type of medication during school hours for the 2005-2006 school year, the following is required:

1. Written order from the physician.
2. Parent/guardian note stating that the school may administer the medication.
3. The medication must come in its original prescription bottle.

Forms for the above may be obtained from the school nurse. This policy also applies to over the counter medications (i.e., cough syrup, etc.) or for any medication that is required during school hours. A separate form will be sent out to all students for the administration of Tylenol for headaches during school hours.

If your child uses an inhaler at home, school, or both, please notify the Health Officer and the nurse will forward a special form for inhaler use. If you have any questions, please contact our school nurse, Mrs. Louise Lavallee, R.N., at 624-4811.
